How do DOACs actually compare with warfarin on outcomes?

Direct oral anticoagulants show efficacy at least comparable to warfarin for stroke prevention in atrial fibrillation, with a generally lower risk of intracranial bleeding across pooled trial and meta-analysis data. Warfarin needs regular INR testing and dose titration, which DOACs largely avoid.

A meta-analysis in elderly atrial fibrillation patients found DOACs at least as effective as warfarin in preventing stroke and systemic embolism, without an increase in major bleeding (comparative efficacy meta-analysis, PMC, NIH). This evidence base is now large enough that most current international guidelines favour a DOAC over warfarin for non-valvular atrial fibrillation where cost and renal function allow it.

DOACs are specifically not recommended in patients with mechanical heart valves or significant mitral stenosis, populations where warfarin remains standard of care based on trial data showing worse outcomes with DOACs in these groups. This is one of the few settings where the newer class is the wrong default, not the better one.

Why does warfarin still dominate volume in India despite the evidence?

Warfarin's tablet cost runs a small fraction of a DOAC's daily cost in most Indian retail and hospital pharmacies, which keeps it the practical default across public-sector and lower-income private care despite the monitoring burden. INR testing infrastructure, imperfect as it is, already exists at scale across Indian district and tertiary hospitals.

A patient on warfarin needs periodic INR checks, typically more frequent at initiation and tapering to monthly once stable, and dietary vitamin K consistency matters for keeping levels in range. Missed monitoring visits are the single most common reason a stable warfarin patient ends up outside therapeutic range and at real risk, in either direction.

Cost-effectiveness modelling in India-specific contexts generally favours warfarin at the population level given current DOAC pricing, while individual-patient decisions still favour a DOAC where monitoring access is poor, adherence to visits is unreliable, or the drug interaction profile of warfarin is a specific problem for that patient.

What monitoring does each class actually require?

Warfarin requires periodic INR testing with a target range set by indication, most commonly 2.0-3.0 for atrial fibrillation and venous thromboembolism, and higher for certain mechanical valves. This is the defining operational burden of the drug and the reason many patients are switched away from it once cost allows.

DOACs need no routine coagulation monitoring in most patients, but periodic renal function testing is still required since all currently marketed DOACs are renally cleared to varying degrees and dose adjustment or avoidance is needed as kidney function declines. What are the real risks a clinician should track across all classes?

Bleeding is the shared, defining risk across every anticoagulant class, ranging from minor bruising to major gastrointestinal or intracranial haemorrhage, and risk scoring tools exist precisely to weigh this against the thromboembolic risk being treated. No anticoagulant decision is risk-free in either direction.

Drug and dietary interactions differ sharply by class. Warfarin interacts with a long list of drugs and with dietary vitamin K intake, while DOACs interact mainly with strong CYP3A4 or P-glycoprotein inhibitors and inducers, a narrower but still clinically significant list. Herbal supplements, particularly those with antiplatelet or hepatic-enzyme effects, are an under-recognised interaction risk with both classes.

Perioperative management, meaning when and how to hold an anticoagulant before a planned procedure and when to restart it after, is a recurring real-world failure point. Both premature discontinuation and unsafe continuation through surgery carry documented harm, and the decision belongs to the treating physician in coordination with the surgical team.
