Antidepressant Names in India: SSRIs, SNRIs, TCAs

A doctor-referenced guide to antidepressant names prescribed in India: SSRIs, SNRIs, TCAs and atypicals, with NLEM and NPPA price-control status from primary sources.

Dr. Priya Menon7 min read
Antidepressant names prescribed in Indian hospitals fall into a handful of working classes: SSRIs (sertraline, escitalopram, fluoxetine, paroxetine), SNRIs (venlafaxine, duloxetine), the TCA amitriptyline, and atypicals such as mirtazapine and bupropion. Only escitalopram, fluoxetine and amitriptyline sit on India's National List of Essential Medicines 2022. Only escitalopram currently carries an NPPA ceiling price.

This covers the generic and Indian brand antidepressant names doctors actually write for each class, which molecules sit on the essential medicines list and NPPA's price schedule, and what that split means for a hospital pharmacy's stocking decisions.

Which SSRI antidepressant names are prescribed in India?

Sertraline, escitalopram, fluoxetine and paroxetine are the four SSRIs most commonly written across Indian outpatient psychiatry. Escitalopram alone is sold under brand names including Nexito (Sun Pharma), Recita (Ipca Laboratories) and Cipralex (Lundbeck India), among more than 60 branded formulations tracked in NPPA's own market data.

That NPPA dataset, compiled from July 2022 sales figures under the Drug Price Control Order 2013, shows Nexito 5 mg tablets alone commanded a 45.41% share of the national escitalopram 5 mg market. Nearly half of every escitalopram tablet sold in that strength was one brand, made by one manufacturer NPPA ceiling price computation, Escitalopram. Sertraline does not appear on the essential medicines list at all.

Fluoxetine is unusual among these antidepressant names. It is listed in NLEM 2022 twice: once as an antidepressant, and separately for obsessive-compulsive disorder, a dual entry no other SSRI in the list carries National List of Essential Medicines 2022 of India: Perspectives from Psychiatrists. Paroxetine sits outside NLEM entirely, alongside sertraline, despite steady clinical use.

What SNRI antidepressant names are stocked in Indian pharmacies?

Venlafaxine and duloxetine are the two SNRIs in routine use across Indian psychiatry and pain clinics. Both are sold under numerous domestic brand names, but neither molecule appears in India's National List of Essential Medicines 2022. The psychiatric disorders section of NLEM 2022 names exactly three antidepressants: amitriptyline, escitalopram and fluoxetine. Venlafaxine and duloxetine are not among them National List of Essential Medicines 2022 of India.

That absence carries a direct pricing consequence. NPPA fixes a DPCO ceiling price only for formulations named in the essential medicines schedule. Venlafaxine and duloxetine brands set their own retail prices without a government ceiling, unlike escitalopram Drug Price Control Order 2013 ceiling price fixation. Duloxetine's dual use for depression and neuropathic or musculoskeletal pain means orthopaedic and pain-management units stock it too, not psychiatry departments alone. That is one more reason to track it as a named line item rather than a generic "SNRI" bucket.

Why does amitriptyline remain India's most consistently listed TCA?

Amitriptyline is the only tricyclic antidepressant named in India's National List of Essential Medicines, holding that place continuously since NLEM 2011 through the 2022 revision. Imipramine appeared in 2011 but was dropped by 2022 National List of Essential Medicines 2022 of India: Perspectives from Psychiatrists.

A 2022 cost-minimization study comparing equivalent antidepressant dosing found amitriptyline's annual treatment cost at ₹2,765.53, close to escitalopram's ₹2,914.79 and well above fluoxetine's ₹1,422.72 Cost-minimization analysis of escitalopram, fluoxetine, and amitriptyline in depression. Amitriptyline's continued NLEM listing and long manufacturing history still keep it stocked in nearly every Indian hospital pharmacy, general practice included.

Amitriptyline is also written outside psychiatry for neuropathic pain, migraine prophylaxis and chronic tension headache. That is one reason its consumption volumes stay high even as newer antidepressant names dominate fresh depression prescriptions.

What atypical antidepressant names (mirtazapine and bupropion) are used in India?

Mirtazapine and bupropion are both marketed in India by domestic manufacturers, but neither appears anywhere on the National List of Essential Medicines 2022, unlike amitriptyline, escitalopram and fluoxetine National List of Essential Medicines 2022 of India. That single fact explains most of the pricing and stocking behaviour around both drugs.

Without an NLEM listing, neither molecule qualifies for an NPPA ceiling price under the Drug Price Control Order 2013. Brand-to-brand pricing for mirtazapine and bupropion is set entirely by manufacturers rather than a government schedule NPPA, Drug Price Control Order 2013. Mirtazapine is reasonably common as an add-on for sleep-disturbed depression. Bupropion is stocked far less widely, and a pharmacy that hasn't confirmed local availability before a prescription is written risks sending a patient chasing one brand across multiple chemists.

Which antidepressant names sit on India's essential medicines list, and which don't?

Three antidepressant names, amitriptyline, escitalopram and fluoxetine, are on India's National List of Essential Medicines 2022, the government's core list of drugs every public health facility is expected to stock National List of Essential Medicines 2022. Escitalopram's inclusion is new to the 2022 edition; it did not appear in NLEM 2015.

Sertraline, paroxetine, venlafaxine, duloxetine, mirtazapine and bupropion sit entirely outside this list despite wide clinical use. For a hospital pharmacy, that split matters operationally more than clinically. NLEM-listed antidepressant names are more likely to carry price ceilings and appear in government procurement rate contracts, while non-listed antidepressants depend on ordinary market distribution alone.

Does NPPA price control actually apply to antidepressant brands?

Escitalopram is the one antidepressant with a live NPPA ceiling price. NPPA's own July 2022 computation under Para 4 of the Drug Price Control Order 2013 fixed escitalopram 5 mg tablets at a ceiling price of ₹4.17 per tablet, notified under S.O. 1499(E) dated 30 March 2022 NPPA ceiling price computation, Escitalopram.

That ceiling exists because escitalopram is scheduled in the DPCO 2013 formulation list. Amitriptyline and fluoxetine, though also on NLEM, do not appear in this same ceiling-price computation, and every other class of antidepressant names covered here sits outside DPCO scheduling altogether. A separate published analysis of DPCO-scheduled antidepressant formulations found compliance was inconsistent even where ceilings exist: 84.6% of the 556 brands studied respected NPPA's ceiling price, while 15.4% were priced above it Cost comparison of commonly used antidepressant drugs with Drug Price Control Order in India.

The practical read for procurement: "price-controlled antidepressant" describes escitalopram specifically, not the category as a whole. Even that single ceiling isn't universally honoured at brand level.

What should a hospital pharmacy do about antidepressant brand names?

A hospital pharmacy stocking psychiatric medication needs to track antidepressant names by brand, not just by molecule, because switching a stabilised patient's brand mid-therapy is not a neutral swap. Adherence in psychiatric care depends heavily on the patient recognising their own tablet.

An unplanned brand change at the counter is a common, avoidable reason patients stop refilling. Reliable in-house stocking of the specific brand a patient started on, Nexito rather than "an SSRI," a named duloxetine brand rather than "an SNRI," protects continuity that a generic-class approach to inventory cannot. Medyzen's guides on managed hospital pharmacy services, pharmacy inventory management for expiry and dead stock, and prescription leakage and hospital revenue loss cover how a hospital builds that brand-level stocking discipline, and what it costs a hospital when a psychiatric refill walks out to an outside chemist instead. NPPA's price control across this category is thin: one ceiling price, on one molecule. The stocking decision sits almost entirely with the hospital, not the regulator.

FAQ

Frequently asked questions

Sertraline, escitalopram, fluoxetine and paroxetine (SSRIs), venlafaxine and duloxetine (SNRIs), amitriptyline (a TCA), and mirtazapine or bupropion (atypicals) cover most prescriptions. Of these antidepressant names, only escitalopram, fluoxetine and amitriptyline are named in India's National List of Essential Medicines 2022.

Yes. NPPA's July 2022 computation under the Drug Price Control Order 2013 fixed escitalopram 5 mg tablets at a ceiling price of ₹4.17 per tablet, notified under S.O. 1499(E). No other antidepressant class covered here currently carries an equivalent NPPA ceiling price.

NLEM 2022 names only three of these: amitriptyline, escitalopram and fluoxetine, reflecting the committee's selection criteria rather than clinical prescribing frequency. Sertraline, paroxetine, venlafaxine, duloxetine, mirtazapine and bupropion are all excluded from the current list.

Amitriptyline has stayed on India's essential medicines list continuously since 2011. A 2022 cost-minimization study priced its annual treatment cost at ₹2,765.53, comparable to escitalopram. It also remains widely used for neuropathic pain and migraine prophylaxis, not depression alone.

Nexito, manufactured by Sun Pharma Laboratories, held a 45.41% share of the national escitalopram 5 mg tablet market in NPPA's own July 2022 sales data, making it the single largest branded escitalopram formulation tracked in that dataset.
