Uses
- Type 2 diabetes in adults when diet, exercise and metformin alone do not control blood sugar.
- Replacing separate vildagliptin and metformin tablets with one combined tablet.
- Used together with a sulfonylurea (like glimepiride) or insulin when more control is needed, on a doctor's advice.
How to take it
Take one tablet twice a day, with or without just after meals (morning and evening), swallowed whole with water – taking it with food reduces the stomach upset from the metformin part. Keep to your diet and exercise plan; the medicine adds to them, it does not replace them. Your doctor will test your liver function before starting and periodically during treatment, and your kidney function at least once a year. If you have vomiting, diarrhoea, fever or cannot eat and drink normally, you can become dehydrated – contact your doctor, as the metformin part may need to be paused. Tell your doctor before any surgery or X-ray dye test.
Side effects
Common
- Nausea, diarrhoea or stomach pain, especially when starting.
- Loss of appetite or a metallic taste.
- Dizziness, headache or tremor.
- Low blood sugar (sweating, shakiness, hunger) if combined with a sulfonylurea or insulin.
- Swelling of the hands or feet.
Serious
Stop and get medical help urgently if you notice any of these.
- Lactic acidosis – vomiting, stomach pain, muscle cramps, difficulty breathing, severe tiredness or cold feeling; rare but an emergency.
- Liver problems – yellowing of the skin or eyes, dark urine, unusual tiredness; stop and see a doctor.
- Severe stomach pain spreading to the back – pancreatitis.
- Blistering skin rash or severe allergic reaction.
Safety information
Alcohol
Avoid excessive alcohol – it sharply increases the risk of lactic acidosis with metformin and can cause low sugar.
Pregnancy
Not recommended in pregnancy or breastfeeding; your doctor will usually switch you to insulin. Tell your doctor if you are planning a pregnancy.
Kidney problems
Not for people with severely reduced kidney function; dose is adjusted in moderate kidney disease and kidney tests are needed at least yearly.
Liver problems
Do not take if you have liver disease; liver enzymes are checked before and during treatment because vildagliptin can rarely affect the liver.
Children
Not recommended under 18 years.
Missed dose
Take it with your next meal unless it is nearly time for the next dose – then skip it. Do not take two doses together.
Interactions
- Other diabetes medicines – glimepiride, gliclazide, insulin – higher risk of low blood sugar; doses may need lowering.
- Water tablets like furosemide and painkillers like ibuprofen, diclofenac – stress the kidneys and raise lactic acidosis risk.
- Steroids and some blood-pressure medicines (ACE inhibitors, ARBs) change blood sugar control or kidney function.
- Iodine contrast dye for CT scans – metformin is usually paused around the test.
- Certain HIV medicines (dolutegravir) and cimetidine raise metformin levels.
Frequently asked questions
What is Galvus Met 50/500 used for?
Galvus Met combines vildagliptin 50 mg and metformin 500 mg in one tablet. It is prescribed for type 2 diabetes when metformin alone, with diet and exercise, does not control blood sugar.
Should I take Galvus Met before or after food?
With or just after a meal, usually breakfast and dinner. Taking it with food reduces nausea and stomach upset from the metformin part and matches the medicine to your blood-sugar rise after eating.
Galvus Met vs Janumet – what is the difference?
Both pair metformin with a DPP-4 inhibitor. Galvus Met uses vildagliptin and is taken twice daily; Janumet uses sitagliptin. They work similarly; vildagliptin needs liver tests during treatment. Your doctor chooses based on cost and your profile.
Does Galvus Met cause low blood sugar?
On its own it rarely causes hypoglycaemia. The risk rises if you also take glimepiride, gliclazide or insulin, or skip meals. Keep sugar or glucose tablets with you and know the warning signs.
Can I drink alcohol with Zomelis Met?
Avoid excessive alcohol. With metformin it raises the risk of lactic acidosis, a rare but life-threatening side effect, and it can also cause low blood sugar.
Why does my doctor check liver tests on Galvus Met?
Vildagliptin can very rarely cause liver inflammation. Liver enzymes are tested before starting and periodically afterwards so any problem is caught early. Report yellowing of the eyes or dark urine immediately.
Does Galvus Met cause weight gain?
No. Metformin tends to be weight-neutral or helps slightly with weight, and vildagliptin does not add weight, unlike some older diabetes medicines.
Can I take Galvus Met if I have kidney disease?
It depends on how well your kidneys work. It is not used if kidney function is severely reduced, and the dose is adjusted in moderate kidney disease. Your doctor will check your kidney test before and during treatment.
Is Galvus Met a prescription medicine?
Yes. It is a Schedule H medicine and needs a doctor's prescription and regular blood sugar, kidney and liver monitoring.
