Uses
- High blood pressure.
- Heart failure – reduces symptoms and hospital admissions.
- Protecting the kidneys in diabetes (diabetic nephropathy).
- Preventing worsening of heart function after a heart attack.
How to take it
Take the tablet once or twice a day as prescribed, at the same times, with or without food. Swallow it whole with water. Your doctor may suggest taking the first dose at bedtime because it can cause dizziness. Potassium and kidney function are checked after starting and after dose increases. If you have vomiting, diarrhoea or fever and cannot drink enough, ask your doctor whether to pause it. Keep taking it even when you feel well – blood pressure has no symptoms.

Safety information
Alcohol
Limit alcohol. It increases dizziness and the fall in blood pressure.
Pregnancy
Do not take in pregnancy – it can seriously harm the baby's kidneys. Tell your doctor immediately if you become pregnant. Enalapril is one of the ACE inhibitors that can often be used while breastfeeding after discussion.
Kidney problems
Used to protect kidneys in diabetes, but the dose is lowered in significant kidney disease and blood tests are essential.
Liver problems
Tell your doctor about liver disease; use with care.
Children
Can be used in children for high blood pressure under a paediatric specialist with weight-based doses.
Missed dose
Take it as soon as you remember unless the next dose is nearly due – then skip it. Never double the dose.
Interactions
- Painkillers like ibuprofen, diclofenac, aspirin – reduce its effect and can harm the kidneys.
- Spironolactone, potassium supplements, salt substitutes – potassium can rise dangerously.
- Furosemide, hydrochlorothiazide and other blood pressure tablets – blood pressure may drop sharply at first.
- Telmisartan, losartan, sacubitril/valsartan – not combined except by a specialist.
- Lithium – levels rise.
- Diabetes medicines – a small risk of lower sugar.
Frequently asked questions
When should I take Envas 5?
At the same time each day, once or twice daily as prescribed. Your doctor may suggest the first dose at bedtime because of dizziness; after that, morning is usual.
Why do I have a dry cough on Envas?
A persistent dry cough is a classic side effect of ACE inhibitors such as enalapril. If it bothers you, your doctor can switch you to an ARB like telmisartan or losartan.
Envas 5 vs Envas 2.5 – what is the difference?
Only the strength. Doctors often start with 2.5 mg, especially in heart failure or older people, and move up to 5 mg or more once blood pressure and kidney tests allow.
Can I take Envas 5 with alcohol?
Keep it minimal. Alcohol adds to the blood pressure drop and dizziness.
Is Enam 5 safe in pregnancy?
No. Enalapril must be stopped as soon as pregnancy is confirmed because it can damage the baby's kidneys. Tell your doctor if you are planning a baby.
Can I take Envas 5 with Aldactone?
Sometimes, in heart failure, but both raise potassium. Blood tests within a week or two of starting are essential, and potassium supplements should be avoided.
Can I take painkillers with Envas 5?
Paracetamol is fine. Regular ibuprofen, diclofenac or aceclofenac reduces its effect and strains the kidneys, especially if you also take a water tablet.
What if I miss a dose of Envas 5?
Take it when you remember unless the next dose is close. Never take two together.
Is enalapril 5 available at Jan Aushadhi?
