Uses

  • Heart failure – added to other heart medicines to reduce fluid and improve survival.
  • Fluid build-up (ascites and leg swelling) due to liver cirrhosis.
  • Resistant high blood pressure that has not come down on other medicines.
  • Primary hyperaldosteronism (too much aldosterone hormone) and low potassium caused by other water tablets.
  • Hormone-related acne, excess facial hair or PCOS in women (specialist use).

How to take it

Take one 25 mg tablet once a day in the morning with or just after food. Swallow it with water. It is a mild water tablet, so the extra urination is gentle but builds up over days. Because it raises potassium, avoid potassium supplements and salt substitutes unless your doctor says otherwise, and have your potassium and kidney function tested as advised. Keep taking it even if you feel well; do not stop without your doctor.

Side effects

Common

  • Dizziness or light-headedness.
  • Nausea.
  • Breast enlargement or tenderness, including in men.
  • Tiredness.
  • Leg cramps.
  • Irregular periods or reduced sex drive.

Serious

Stop and get medical help urgently if you notice any of these.

  • High potassium – muscle weakness, slow or irregular heartbeat, numbness; can be dangerous.
  • Kidney problems – very little urine, swelling, confusion.
  • Severe skin reaction – blistering, peeling rash.
  • Yellowing of the eyes or skin (liver problem).

Safety information

Alcohol

Alcohol adds to dizziness and the drop in blood pressure. Keep it minimal.

Pregnancy

Not usually recommended in pregnancy as it can affect the baby's development. Tell your doctor if you are pregnant or planning to be. Breastfeeding may be possible after discussion.

Kidney problems

Not suitable in severe kidney disease because potassium can rise dangerously. Regular blood tests are essential even with mild kidney disease.

Liver problems

Commonly used in cirrhosis, but salts must be monitored closely.

Children

Used in children for heart and kidney conditions with weight-based dosing under a specialist.

Missed dose

Take it as soon as you remember unless the next dose is nearly due – then skip it. Do not double the dose.

Interactions

  • Ramipril, enalapril, telmisartan, losartan – together they can push potassium too high; blood tests are needed.
  • Potassium tablets, ORS with high potassium, salt substitutes like Lo-Salt – avoid unless prescribed.
  • Painkillers like ibuprofen, diclofenac, aceclofenac – raise potassium and strain the kidneys.
  • Digoxin – spironolactone can raise its level and interfere with its blood test.
  • Trimethoprim (co-trimoxazole), heparin, cyclosporine – more potassium rise.
  • Lithium – levels can rise.

Frequently asked questions

When should I take Aldactone 25?

In the morning with or after food. It is a gentle water tablet, so you will not rush to the toilet the way you would with Lasix.

Can I take Aldactone with Lasix?

Yes, they are often prescribed together in heart failure and cirrhosis. Lasix loses potassium and Aldactone saves it, so the combination balances out – but blood tests are still needed.

Does Aldactone cause breast enlargement in men?

Yes, breast swelling or tenderness (gynaecomastia) is a known side effect because it blocks male hormones. It usually goes away after stopping. Tell your doctor; eplerenone is an alternative.

Can I eat bananas or coconut water on Aldactone 25?

In moderation. Spironolactone raises potassium, so very high-potassium foods, potassium salt substitutes and supplements should be limited. Ask your doctor or dietitian for a sensible limit.

Is Aldactone safe in pregnancy?

It is generally avoided in pregnancy. Tell your doctor if you are pregnant or planning a baby so a safer medicine can be chosen.

Can I take Aldactone 25 with alcohol?

Keep alcohol minimal. It worsens dizziness and low blood pressure, and in liver disease alcohol must be avoided entirely.

Can Aldactone be used for acne or PCOS?

Dermatologists and gynaecologists sometimes prescribe spironolactone for hormonal acne, hirsutism or PCOS. It needs contraception because of risks to a male baby, and potassium checks.

Can I take Aldactone with Telma or ramipril?

Often yes, especially in heart failure, but all three raise potassium. Your doctor will check potassium and creatinine within a week or two of starting and periodically after.

Is spironolactone 25 available at Jan Aushadhi?

Yes, spironolactone 25 mg is listed under the Jan Aushadhi scheme and is NPPA price-controlled. A prescription is needed.
