Uses

  • High blood pressure (hypertension).
  • Lowering the risk of heart attack and stroke in people over 55 with heart disease or diabetes with organ damage.
  • Protecting the kidneys in people with diabetes and high blood pressure, as decided by a doctor.

How to take it

Take one 40 mg tablet once a day at the same time each day, with or without food, swallowed with water. It takes 4 to 8 weeks for the full blood pressure effect, so keep taking it even when you feel fine. Do not stop suddenly without your doctor's advice.

Side effects

Common

  • Dizziness, especially when standing up.
  • Back pain.
  • Sinus or upper respiratory infection symptoms.
  • Diarrhoea.

Serious

Stop and get medical help urgently if you notice any of these.

  • Swelling of the face, lips, tongue or throat (angioedema) – emergency.
  • Fainting, very low blood pressure.
  • High potassium – muscle weakness, irregular heartbeat.
  • Reduced kidney function – less urine, swelling of the legs.

Safety information

Alcohol

Alcohol lowers blood pressure further and can cause dizziness or fainting with telmisartan. Limit it.

Pregnancy

Do not take in pregnancy – telmisartan can seriously harm the unborn baby, especially after the first 3 months. Stop and call your doctor immediately if you become pregnant. Not recommended while breastfeeding.

Kidney problems

Tell your doctor about kidney disease; kidney function and potassium are checked regularly. Avoid if you have narrowing of both kidney arteries.

Liver problems

Use with caution in liver disease; your doctor may choose a lower dose.

Children

Not routinely used in children.

Missed dose

Take it as soon as you remember unless the next dose is within 12 hours; then skip it. Do not double up.

Interactions

  • Potassium supplements, potassium salt substitutes and potassium-sparing diuretics (spironolactone) – high potassium.
  • ACE inhibitors (ramipril, enalapril) or aliskiren – combination not recommended, especially in diabetes.
  • NSAID painkillers (ibuprofen, diclofenac, aceclofenac) – reduce the BP effect and can harm the kidneys.
  • Lithium – levels can rise.
  • Other BP medicines and water pills – additive lowering of blood pressure (often intended, but watch for dizziness).

Frequently asked questions

Telma 40 morning or night?

Either is fine as long as you take it at the same time every day. Some doctors suggest night-time dosing for better overnight control; follow your doctor's advice.

Can I take Telma 40 with alcohol?

Alcohol can increase the blood pressure-lowering effect and cause dizziness or fainting. Keep alcohol minimal and avoid it when starting the medicine.

Can I take Telma 40 on an empty stomach?

Yes. Telmisartan can be taken with or without food.

Is Telma 40 safe in pregnancy?

No. Telmisartan can cause serious harm to the baby. If you are planning pregnancy or find out you are pregnant, contact your doctor straight away to switch to a safer medicine.

Can I stop Telma 40 once my BP is normal?

No. Your BP is normal because of the medicine. Stopping it will let your blood pressure rise again. Only your doctor should reduce or stop it.

Telma 40 vs Telma H – what is the difference?

Telma 40 has only telmisartan 40 mg. Telma H adds hydrochlorothiazide 12.5 mg, a water pill, for people whose BP is not controlled on telmisartan alone.

Can I take painkillers like Zerodol or Brufen with Telma 40?

Occasional use is usually tolerated, but regular NSAID painkillers reduce telmisartan's effect and can harm the kidneys. Paracetamol is the safer choice; ask your doctor for anything beyond a day or two.

Does Telma 40 affect the kidneys?

Telmisartan usually protects the kidneys in diabetes and hypertension, but in people with already-weak kidneys or dehydration it can reduce kidney function. Regular blood tests for creatinine and potassium are part of safe use.

Does Telma 40 need a prescription?

Yes. Telmisartan is a Schedule H medicine in India and must be prescribed by a doctor.
