Uses
- High blood pressure that is not controlled by losartan alone.
- High blood pressure where a water tablet (diuretic) is also needed, for example with mild fluid retention.
- Reducing stroke risk in people with high blood pressure and a thickened heart muscle.
How to take it
Take one tablet once a day, preferably in the morning so the water-tablet part does not keep you up at night, with or without food, swallowed whole with water. It is a long-term treatment; your doctor will check your blood pressure, kidney tests, sodium and potassium regularly. Do not stop without medical advice. If you have vomiting, diarrhoea or heavy sweating in hot weather you can get dehydrated quickly on this combination – drink fluids and ask your doctor whether to pause it. Get up slowly from sitting or lying to avoid dizziness.
Side effects
Common
- Dizziness or light-headedness, especially on standing.
- Passing urine more often.
- Tiredness or headache.
- Muscle cramps.
- Skin more sensitive to sunlight.
Serious
Stop and get medical help urgently if you notice any of these.
- Swelling of the face, lips or throat (angioedema) – get emergency help.
- Severe dehydration – dry mouth, intense thirst, very little urine, confusion, fast heartbeat.
- Irregular heartbeat or muscle weakness – a sign of abnormal potassium or sodium.
- Yellowing of the eyes, severe stomach pain, or a sudden eye pain with blurred vision.
Safety information
Alcohol
Avoid alcohol – it worsens dizziness and dehydration and lowers blood pressure further.
Pregnancy
Do not take in pregnancy – losartan can seriously harm the baby. Tell your doctor at once if you become pregnant. Not recommended while breastfeeding.
Kidney problems
Kidney function and electrolytes must be checked; it is not suitable if your kidneys are severely weak.
Liver problems
Not suitable in severe liver disease, and used with caution in milder liver problems.
Children
Not recommended for children.
Missed dose
Take it as soon as you remember unless the next dose is nearly due – then skip it. Never double the dose.
Interactions
- Potassium supplements or salt substitutes and spironolactone – potassium may go too high or too low; needs monitoring.
- Painkillers like ibuprofen, diclofenac and aceclofenac – reduce the effect and stress the kidneys.
- Lithium – levels rise with both losartan and hydrochlorothiazide; usually avoided.
- Diabetes medicines – the thiazide part can raise blood sugar slightly; sugar may need rechecking.
- Gout medicines (allopurinol, febuxostat) – hydrochlorothiazide can raise uric acid and trigger gout.
- Steroids and digoxin – the thiazide can lower potassium and increase digoxin side effects.
Frequently asked questions
What is Losar-H used for?
Losar-H contains losartan 50 mg plus hydrochlorothiazide 12.5 mg, a mild water tablet. It is prescribed when losartan alone does not bring blood pressure down enough.
Losar-H vs Losar 50 – which should I take?
Losar 50 is plain losartan; Losar-H adds a diuretic for extra blood-pressure lowering. Only switch on your doctor's advice, because the combination needs kidney and potassium monitoring.
Should I take Repace H in the morning or evening?
Morning is usually best, because the hydrochlorothiazide part makes you pass more urine for a few hours and a night dose can disturb sleep.
Can I take Losar-H in summer or during a stomach infection?
Yes, but be careful. Heavy sweating, vomiting or diarrhoea plus a water tablet can cause dehydration and a drop in blood pressure. Drink enough fluids and ask your doctor if you should pause the tablet while unwell.
Is Losar-H safe in pregnancy?
No. Losartan can seriously harm an unborn baby and the combination is not used in pregnancy or breastfeeding. Tell your doctor immediately if you are pregnant or planning to be.
Can I take Losar-H with alcohol?
It is best avoided. Alcohol increases dizziness, dehydration and the blood-pressure-lowering effect of both components.
Does Losar-H affect blood sugar or uric acid?
The hydrochlorothiazide part can slightly raise blood sugar and uric acid. If you have diabetes or gout, your doctor will monitor these and may choose a different combination.
Can I take painkillers with Losar-H?
Use paracetamol. Regular ibuprofen, diclofenac or other NSAIDs reduce the effect of this medicine and can harm the kidneys when combined with it.
Is Losar-H a prescription medicine?
Yes. It is a Schedule H medicine in India and must be dispensed only against a doctor's prescription, with regular blood tests during treatment.
