Uses
- High blood pressure that is not controlled with telmisartan alone.
- High blood pressure with fluid retention (swelling), where a water pill helps.
- Reducing heart and stroke risk by keeping BP in range, under a doctor's care.
How to take it
Take one tablet once a day at the same time each day, with or without food. Many people take it in the morning because the hydrochlorothiazide part makes you pass more urine for a few hours. Keep taking it even when you feel well; the full effect takes several weeks.
Side effects
Common
- Dizziness or light-headedness, especially on standing.
- Passing urine more often.
- Back pain, tiredness.
- Low potassium or sodium (muscle cramps, weakness).
- Raised blood sugar or uric acid (gout).
Serious
Stop and get medical help urgently if you notice any of these.
- Swelling of the face, lips or throat (angioedema).
- Fainting or very low blood pressure.
- Severe dehydration – very dry mouth, extreme thirst, confusion.
- Sudden eye pain or vision change (acute glaucoma from the thiazide).
- Reduced kidney function.
Safety information
Alcohol
Alcohol increases dizziness and dehydration with this combination. Limit it.
Pregnancy
Do not take in pregnancy – telmisartan can harm the unborn baby. Stop and contact your doctor if you become pregnant. Not recommended while breastfeeding.
Kidney problems
Not suitable if you have severe kidney disease; kidney function and electrolytes need regular checks.
Liver problems
Avoid in severe liver disease. Tell your doctor about any liver problem.
Children
Not used in children.
Missed dose
Take it as soon as you remember unless the next dose is within 12 hours; then skip it. Do not double up.
Interactions
- Potassium supplements or spironolactone – potassium can go too high or swing; needs monitoring.
- Lithium – levels can rise dangerously.
- NSAID painkillers – reduce the BP effect and strain the kidneys.
- Diabetes medicines – the thiazide may raise blood sugar; doses may need adjusting.
- Other BP medicines and water pills – additive effects, dizziness.
- Digoxin and medicines affecting heart rhythm – low potassium increases the risk of rhythm problems.
Frequently asked questions
Telma H morning or night?
Morning is usual, because the hydrochlorothiazide makes you urinate more in the first few hours and taking it late can disturb sleep. Keep the same time every day.
Can I take Telma H with alcohol?
Avoid or limit alcohol – it increases dizziness and dehydration with the water pill.
Telma H vs Telma 40 – which is stronger?
Telma H contains telmisartan 40 mg plus the diuretic hydrochlorothiazide 12.5 mg, so it lowers BP more than Telma 40 alone. It is usually prescribed when telmisartan alone is not enough.
Does Telma H cause frequent urination?
Yes, mildly, especially in the first few hours after the dose, because of hydrochlorothiazide. This usually becomes less noticeable over time.
Is Telma H safe in pregnancy?
No. The telmisartan component can harm the unborn baby. Tell your doctor immediately if you are pregnant or planning pregnancy.
Can Telma H raise my blood sugar or uric acid?
Hydrochlorothiazide can slightly raise blood sugar and uric acid in some people. If you have diabetes or gout, your doctor will monitor these.
Can I take Telma H on an empty stomach?
Yes, it can be taken with or without food.
Does Telma H need a prescription?
Yes. Both components are Schedule H medicines in India and need a doctor's prescription and monitoring.
