Uses

  • Helping you stop smoking cigarettes or bidis by relieving nicotine cravings and withdrawal.
  • Helping you stop chewing tobacco, gutkha or khaini.
  • Cutting down smoking step by step before quitting completely.
  • Managing cravings in places where you cannot smoke (work, travel, hospital).

How to take it

Use the 2 mg gum if you smoke 20 or fewer cigarettes/bidis a day (heavier smokers need the 4 mg). Do not chew it like ordinary gum. Chew one piece slowly until you feel a strong taste or tingling, then "park" it between your cheek and gum; when the taste fades, chew again. Repeat this chew-and-park cycle for about 30 minutes, then throw it away. Use one piece whenever you get a craving, usually 8 to 12 pieces a day, and never more than 15 in a day. Avoid coffee, tea, cold drinks and acidic juices for 15 minutes before and during use because they stop nicotine being absorbed. Use for about 3 months, then gradually reduce the number of pieces over the next few weeks until you stop. Combining the gum with counselling (National Tobacco Quitline 1800-11-2356) doubles your chances. Do not smoke while using it.

Side effects

Common

  • Hiccups, throat irritation, sore mouth – usually from chewing too fast.
  • Jaw ache.
  • Indigestion, heartburn, nausea.
  • Headache, dizziness.
  • Increased salivation.

Serious

Stop and get medical help urgently if you notice any of these.

  • Fast or irregular heartbeat, chest palpitations – stop and see a doctor.
  • Severe allergic reaction – swelling of the mouth, face or throat, difficulty breathing.
  • Signs of too much nicotine – feeling faint, cold sweat, vomiting, severe headache.
  • Child swallowing the gum – nicotine poisoning can be fatal in children; keep it locked away.

Safety information

Alcohol

No direct interaction, but alcohol is a common smoking trigger – avoid it in the first weeks of quitting.

Pregnancy

Quitting without medicine is best, but if you cannot, nicotine gum is far safer than continuing to smoke; use intermittent products like gum rather than patches and ask your doctor. If breastfeeding, feed the baby first and then use the gum.

Kidney problems

Tell your doctor about severe kidney disease; nicotine clearance may be slower.

Liver problems

Tell your doctor about severe liver disease.

Children

Not for anyone under 12. Nicotine in high doses can kill a small child – store out of reach.

Missed dose

There is no fixed schedule – use a piece when you have a craving. Do not use extra pieces together.

Interactions

  • Coffee, tea, cola and acidic juices – reduce absorption; avoid 15 minutes before and during use.
  • Theophylline (asthma), clozapine, ropinirole and some other medicines – stopping smoking changes their levels, so doses may need adjusting; tell your doctor you are quitting.
  • Insulin and diabetes tablets – blood sugar control may change after quitting; monitor.
  • Other nicotine products – patches plus gum can be combined only under a doctor's guidance; never smoke while using the gum.
  • Blood pressure and heart medicines – tell your doctor if you have recent heart attack, angina or irregular heartbeat before starting.

Frequently asked questions

How do I use 2baconil or Nicotex gum correctly?

Chew slowly until the taste becomes strong, then park the gum between your cheek and gums. When the taste fades, chew again. Keep this up for about 30 minutes, then discard. Chewing it continuously like normal gum releases nicotine too fast and causes hiccups, sore throat and nausea.

Nicotine gum 2 mg or 4 mg – which one should I use?

Use 2 mg if you smoke 20 or fewer cigarettes or bidis a day, or chew tobacco occasionally. Use 4 mg if you smoke more than 20 a day, smoke within 30 minutes of waking, or if the 2 mg gum does not control your cravings.

How many nicotine gums can I use in a day?

Most people need 8 to 12 pieces a day in the first weeks. Never use more than 15 pieces in 24 hours, and use only one piece at a time.

How long should I use nicotine gum?

Use it for about 3 months, then gradually reduce the number of pieces a day over several weeks until you no longer need it. If you still need the gum after 6 to 9 months, talk to a doctor.

Can I smoke while using nicotine gum?

No. Smoking while using the gum gives you too much nicotine – palpitations, nausea, dizziness – and defeats the purpose. Set a quit date and use the gum instead of cigarettes from that day.

Why do I get hiccups and a sore throat with the gum?

You are chewing too fast, which releases nicotine quickly and makes you swallow it. Chew slowly, park the gum between the cheek and gum, and chew again only when the taste fades.

Can I use nicotine gum to quit gutkha or khaini?

Yes. Nicotine gum works for smokeless tobacco too, since the craving is for nicotine. The chew-and-park method also replaces the habit of keeping something in the mouth. Combine it with counselling for the best results.

Is nicotine gum safe in pregnancy?

The best option is to quit without medicine, but if you cannot, nicotine gum is much safer than smoking for both mother and baby. Prefer intermittent products such as gum over patches, and discuss it with your doctor. If breastfeeding, feed the baby just before using a piece.

Why does coffee reduce the effect of nicotine gum?

Nicotine is absorbed through the lining of the mouth, and acidic drinks – coffee, tea, cola, fruit juice – lower the mouth's pH and block this absorption. Avoid them for 15 minutes before and while using the gum.

Is nicotine gum available without prescription in India?

Yes. Nicotine gum (2baconil, Nicotex, Nicosure, Nicogum) is sold over the counter at chemists and online, and 2 mg gum is also listed at Jan Aushadhi stores. It is not for anyone under 12.
