Uses
- Acid reflux (GERD) with heartburn and regurgitation.
- Acidity with nausea, vomiting, bloating or a feeling of fullness after meals.
- Indigestion (dyspepsia) where the stomach empties slowly.
- Gastritis and peptic ulcer symptoms, as prescribed by your doctor.
How to take it
Take one capsule once a day, 30 to 60 minutes before breakfast, with water. Swallow it whole – the capsule contains coated pellets that must not be chewed or opened. Use it only for the duration your doctor prescribes; domperidone is normally limited to the shortest course needed (often 1 week) and the lowest effective dose.
Side effects
Common
- Headache.
- Dry mouth.
- Diarrhoea or constipation.
- Stomach pain, gas or bloating.
- Dizziness.
Serious
Stop and get medical help urgently if you notice any of these.
- Irregular or fast heartbeat, fainting – domperidone can affect heart rhythm, especially in people over 60 or on other heart medicines. Stop and seek help.
- Breast enlargement or milk discharge, missed periods (domperidone raises prolactin).
- Severe allergic reaction – swelling of face or throat, breathing difficulty.
- Persistent severe diarrhoea.
Safety information
Alcohol
Avoid alcohol. It worsens reflux and can add to dizziness and drowsiness with domperidone.
Pregnancy
Not recommended in pregnancy unless your doctor says the benefit outweighs the risk. Domperidone passes into breast milk in small amounts; ask your doctor before using while breastfeeding.
Kidney problems
Tell your doctor if you have kidney disease; domperidone may need a lower dose or less frequent dosing.
Liver problems
Do not use if you have moderate or severe liver disease – domperidone is not recommended in that case.
Children
Not for children; the strengths in this combination are adult doses.
Missed dose
Take it when you remember unless the next dose is nearly due. Never take two capsules together.
Interactions
- Medicines that affect heart rhythm – some antibiotics (erythromycin, clarithromycin), antifungals (ketoconazole, fluconazole), antidepressants and antipsychotics. Combining them with domperidone can cause dangerous rhythm problems.
- Ketoconazole, itraconazole and other strong CYP3A4 blockers raise domperidone levels – avoid.
- Medicines that need stomach acid (iron, some antifungals, atazanavir) may be absorbed less.
- Warfarin and methotrexate – levels may change; monitoring may be needed.
- Anticholinergic medicines (such as some antispasmodics) can reduce domperidone's effect on the gut.
Frequently asked questions
Pan D before or after food?
Before food. Take Pan D 30 to 60 minutes before breakfast on an empty stomach so both medicines work before you eat.
Can I take Pan D daily?
Take it once a day only for as long as your doctor prescribes. Domperidone should not be used for long periods because of heart-rhythm risk; if you still need an acid medicine after a short course, your doctor may switch you to plain pantoprazole.
Pan D vs Pan 40 – which is better for gas?
Pan D includes domperidone, which speeds up stomach emptying and helps with bloating, nausea and fullness. Pan 40 only reduces acid. If gas and nausea are your main problem, doctors usually prefer the combination for a short course.
Can I take Pan D with alcohol?
Better not. Alcohol worsens acidity and may increase drowsiness and dizziness. It also undoes the benefit of the medicine.
Is Pan D safe in pregnancy?
It is generally avoided in pregnancy unless your doctor considers it essential. Discuss safer options with your doctor.
Does Pan D need a prescription?
Yes. This combination is a Schedule H medicine in India and should only be sold against a prescription.
Why do I have to swallow Pan D whole?
The capsule contains enteric-coated pantoprazole and slow-release domperidone pellets. Chewing or opening it destroys the coating and the medicine may not work properly.
Can Pan D cause heart problems?
Domperidone can prolong the heart's QT interval, especially at higher doses, in older people and with certain other medicines. Tell your doctor about any heart condition or if you feel palpitations or faint.
