Uses
- Infertility in women who do not ovulate regularly, including polycystic ovary syndrome (PCOS).
- Inducing ovulation before timed intercourse or intrauterine insemination (IUI).
- Sometimes used off-label in men with low sperm count or low testosterone, under a specialist.
How to take it
Take one 50 mg tablet once a day for 5 days, starting on the day of your cycle your doctor tells you (commonly day 2 to day 5 of the period). Swallow whole with water, at the same time each day, with or without food. Ovulation usually happens 5 to 10 days after the last tablet, and your doctor may track it with ultrasound (follicular monitoring) and advise when to have intercourse. If you do not ovulate, the doctor may raise the dose to two tablets a day in the next cycle. Treatment is usually limited to 3 to 6 cycles; do not continue month after month without a review. Do a pregnancy test before each new course.
Side effects
Common
- Hot flushes.
- Bloating, lower abdominal discomfort or ovarian pain.
- Breast tenderness.
- Nausea, headache, mood swings.
- Spotting between periods or heavier periods.
Serious
Stop and get medical help urgently if you notice any of these.
- Ovarian hyperstimulation (OHSS) – severe pelvic pain, rapid weight gain, swollen abdomen, breathlessness, passing little urine – go to hospital.
- Visual disturbances – blurring, spots, flashes or double vision – stop the tablets and see the doctor; this can be permanent if ignored.
- Multiple pregnancy (twins or more) – higher risk of complications.
- Severe headache, one-sided weakness or slurred speech (rare stroke), or yellowing of the eyes.
Safety information
Alcohol
Alcohol does not directly interact, but it lowers fertility and should be avoided while trying to conceive.
Pregnancy
Do not take if you are already pregnant – confirm with a test before each course. Clomiphene can reduce breast milk; tell your doctor if you are breastfeeding.
Kidney problems
No specific dose change; tell your doctor about kidney disease.
Liver problems
Do not take if you have liver disease; clomiphene is processed by the liver.
Children
Not for children or adolescents.
Missed dose
Contact your doctor – the cycle may need adjusting. Do not double the next dose.
Interactions
- Other fertility medicines (letrozole, gonadotropin injections like HMG/FSH) – only in a planned protocol; combining increases OHSS risk.
- Hormone treatments and the contraceptive pill – conflicting effects; stop the pill before a clomiphene cycle as your doctor advises.
- Metformin (for PCOS) – often combined deliberately; this is fine under supervision.
- Herbal "fertility" supplements with unknown hormones – avoid during a cycle.
Frequently asked questions
What is Siphene 50 used for?
Siphene contains clomiphene citrate 50 mg, a medicine that makes the ovaries release an egg. It is prescribed for women who are not ovulating regularly – most often because of PCOS – to help them conceive.
On which day of the period should I start Siphene 50?
Most doctors start it between day 2 and day 5 of the menstrual cycle (counting the first day of bleeding as day 1) and continue for 5 days. Follow your gynaecologist's exact instructions, because the start day affects follicle growth and timing of ovulation.
When does ovulation happen after Fertomid 50?
Usually 5 to 10 days after the last tablet, so roughly day 12 to day 17 of the cycle. Your doctor may do ultrasound scans to track the follicle and advise the best days for intercourse or IUI.
What is the chance of twins with clomiphene?
Clomiphene raises the chance of twins compared with natural conception because more than one egg may be released. Triplets or more are rare. Your doctor uses the lowest effective dose and monitors follicles to limit this.
How many cycles of Siphene can I take?
Doctors generally limit clomiphene to about 3 to 6 ovulatory cycles. If ovulation happens but pregnancy does not, or if there is no ovulation even at the higher dose, the next step is a different treatment such as letrozole, injections or IVF rather than continuing indefinitely.
Siphene vs letrozole (Letroz) for PCOS – which is better?
Both are used to induce ovulation. Letrozole is now often preferred first in PCOS because it has fewer effects on the uterine lining and a slightly lower twin rate, while clomiphene has decades of experience and is cheaper. Your fertility specialist decides based on your case.
Can clomiphene cause vision problems?
Yes, rarely – blurred vision, spots or flashes of light, especially in bright light. Stop the tablets and tell your doctor at once; vision problems can become permanent if the drug is continued. Do not drive if your vision is affected.
What is OHSS and how do I recognise it?
Ovarian hyperstimulation syndrome is when the ovaries over-respond, swell and leak fluid. Warning signs are severe pelvic pain, a rapidly swelling abdomen, nausea, breathlessness and passing very little urine. It is a medical emergency – go to hospital.
Is Siphene 50 available without prescription?
No. Clomiphene is a Schedule H medicine and should be taken only under a gynaecologist or fertility specialist with proper monitoring. Self-medicating risks OHSS and multiple pregnancy.
