Uses
- Preventing another heart attack or stroke in people who have had one.
- After angioplasty and stent placement, usually with aspirin, to stop the stent clotting.
- Acute coronary syndrome (unstable angina or heart attack), with aspirin.
- Peripheral artery disease, to reduce clot risk.
- Preventing stroke in atrial fibrillation when warfarin or newer blood thinners cannot be used.
How to take it
Take one 75 mg tablet once a day at the same time each day, with or without food. Swallow it whole with water. Your cardiologist may give a one-off higher loading dose at the start. Clopidogrel is usually taken for a few months to a year after a stent, together with aspirin, and sometimes for life – do not stop before the date your cardiologist sets, because stopping early can cause a stent to clot and trigger a heart attack. Tell every doctor and dentist you take it. See a doctor for black stools, blood in urine, unusual bruising, or a bleed that will not stop.
Side effects
Common
- Bleeding more easily – nosebleeds, bruising, longer bleeding from cuts.
- Diarrhoea.
- Stomach pain or indigestion.
- Heavier periods.
Serious
Stop and get medical help urgently if you notice any of these.
- Heavy or internal bleeding – black tarry stools, blood in urine, vomiting or coughing blood – go to hospital.
- Sudden severe headache, confusion or weakness (possible brain bleed) – emergency.
- Fever, extreme tiredness, purple skin spots or yellow skin (rare blood disorder TTP) – urgent.
- Serious allergic reaction – swelling of the face or throat, difficulty breathing.
Safety information
Alcohol
Keep alcohol moderate. Heavy drinking irritates the stomach and raises bleeding risk, especially with aspirin.
Pregnancy
Not usually used in pregnancy or breastfeeding unless a specialist judges it essential; tell your doctor.
Kidney problems
No routine dose change, but tell your doctor about kidney disease; bleeding risk is higher.
Liver problems
Clopidogrel needs the liver to activate it; it is used with caution in liver disease.
Children
Not routinely used in children.
Missed dose
If you are less than 12 hours late, take it as soon as you remember. If more than 12 hours, skip it and take the next dose at the usual time. Never take two doses together.
Interactions
- Omeprazole and esomeprazole – reduce clopidogrel's activation; pantoprazole or rabeprazole is preferred for acidity.
- Painkillers like ibuprofen, diclofenac and high-dose aspirin – higher bleeding risk; paracetamol is safer.
- Other blood thinners – warfarin, apixaban, rivaroxaban, dabigatran – only when a cardiologist plans the combination.
- SSRI antidepressants (sertraline, escitalopram) – increased bleeding.
- Fluconazole, ketoconazole, carbamazepine, rifampicin and St John's wort – change clopidogrel's effect.
- Repaglinide and pioglitazone (diabetes) – levels may rise; tell your doctor.
Frequently asked questions
What is Clopivas 75 used for?
Clopivas 75 contains clopidogrel 75 mg, an antiplatelet medicine. It stops platelets clumping into clots and is used after a heart attack, stroke or stent, and in peripheral artery disease.
Clopivas 75 vs Clopivas AP – what is the difference?
Clopivas 75 is clopidogrel alone. Clopivas AP adds low-dose aspirin in the same tablet (dual antiplatelet therapy), which is the usual combination after a stent. Your cardiologist decides which you need and for how long.
How long do I need to take Deplatt after a stent?
Typically 6 to 12 months together with aspirin, then aspirin alone, but it depends on the type of stent and your bleeding risk. Never stop before the date your cardiologist gives you – stopping early can cause the stent to clot.
Can I take Clopivas 75 with alcohol?
Keep alcohol moderate. Heavy drinking irritates the stomach and increases bleeding risk, especially when you also take aspirin.
Can I take Pan 40 (pantoprazole) with clopidogrel?
Yes. Pantoprazole and rabeprazole are the preferred acidity medicines with clopidogrel. Avoid omeprazole and esomeprazole, which reduce clopidogrel's activation.
Can I take Dolo 650 or Combiflam with Deplatt?
Paracetamol (Dolo) is fine. Avoid Combiflam, ibuprofen and other NSAIDs unless your doctor approves – they add to bleeding risk.
Do I need to stop clopidogrel before surgery or a tooth extraction?
Tell the surgeon or dentist well in advance. Many dental procedures can be done without stopping. For major surgery it is often paused 5 to 7 days before, but only with your cardiologist's agreement, especially if you have a recent stent.
Is Plavix the same as Clopivas or Deplatt?
Yes. Plavix is the original brand; Clopivas, Deplatt, Clopilet and Clopitab are Indian brands of the same clopidogrel 75 mg. Ask your doctor before switching.
I missed a dose of Clopivas 75. What should I do?
If you are less than 12 hours late, take it now. If more than 12 hours have passed, skip it and take the next dose at the usual time. Do not double up.
