Uses

  • Parkinson's disease – reduces tremor, stiffness and slowness of movement.
  • Parkinsonism after brain infection (encephalitis) or carbon monoxide / manganese poisoning.
  • Levodopa is turned into dopamine in the brain; carbidopa stops it being broken down early so a smaller dose works with less nausea.

How to take it

Take the tablets at the same times every day, usually 3 or 4 times a day, exactly as your neurologist has set – the dose is increased slowly over weeks. Swallow with water; the controlled-release (CR) version must be swallowed whole, not crushed. Taking it with a light snack reduces nausea, but a heavy protein meal (dal, meat, milk, paneer) at the same time can block absorption, so many doctors advise taking it 30 to 60 minutes before meals. Never stop suddenly – stopping can cause a dangerous reaction with high fever, rigid muscles and confusion; your doctor will reduce the dose gradually if needed.

Side effects

Common

  • Nausea, loss of appetite, dry mouth.
  • Dizziness or light-headedness on standing up.
  • Vivid dreams, trouble sleeping, confusion or forgetfulness.
  • Dark-coloured urine, sweat or saliva – harmless but can stain clothes.
  • Headache, weakness.

Serious

Stop and get medical help urgently if you notice any of these.

  • Uncontrolled jerky movements of the face, limbs or body (dyskinesia) – the dose may need adjusting.
  • Hallucinations, severe confusion, depression or thoughts of self-harm.
  • Sudden urge to gamble, shop, eat or increased sexual urges (impulse-control problems) – tell the family to watch for these.
  • Falling asleep suddenly during the day, chest pain or irregular heartbeat, black stools or vomiting blood.

Safety information

Alcohol

Avoid or strictly limit alcohol – it worsens drowsiness, dizziness and confusion.

Pregnancy

Tell your doctor if you are pregnant, planning pregnancy or breastfeeding; it is used only when the benefit clearly outweighs the risk.

Kidney problems

Tell your doctor about kidney disease; monitoring may be needed.

Liver problems

Tell your doctor about liver disease; the dose may need review.

Children

Not used in children.

Missed dose

Take it as soon as you remember unless the next dose is nearly due – then skip it. Never double up. Keeping a dose diary helps because timing matters a lot in Parkinson's.

Interactions

  • MAO inhibitors such as selegiline, rasagiline and the antidepressants phenelzine/tranylcypromine – can cause dangerous rises in blood pressure; your neurologist controls these combinations.
  • Iron tablets and iron-containing multivitamins – reduce absorption; take iron at least 2 hours apart.
  • High-protein meals – compete with levodopa for absorption; space the tablet away from large protein meals.
  • Antipsychotics and anti-vomiting drugs like metoclopramide (Perinorm), prochlorperazine (Stemetil), haloperidol – block dopamine and worsen Parkinson's; domperidone is the preferred anti-nausea drug.
  • Blood pressure medicines – added dizziness on standing.
  • Some allergy medicines (diphenhydramine, chlorpheniramine) and sleeping tablets – more drowsiness and confusion.

Frequently asked questions

What is Syndopa Plus used for?

Syndopa Plus contains levodopa 100 mg and carbidopa 25 mg. It is the main medicine for Parkinson's disease, replacing the dopamine the brain is no longer making, which improves tremor, stiffness and slowness. It controls symptoms but does not cure or stop the disease.

Should Syndopa Plus be taken before or after food?

Ideally 30 to 60 minutes before meals, because protein in food (dal, milk, paneer, meat) competes with levodopa and reduces its effect. If it causes nausea, take it with a small non-protein snack such as a biscuit or fruit. Keep the timing the same every day.

Syndopa Plus vs Syndopa CR – what is the difference?

Both contain levodopa and carbidopa. Syndopa Plus is a normal-release tablet that acts within 30 to 60 minutes and lasts a few hours. Syndopa CR releases the medicine slowly, is often used at night or to smooth out "wearing off", and must be swallowed whole. Your neurologist decides the combination.

Why does Tidomet Plus make my urine dark?

Levodopa breaks down into coloured compounds that turn urine, sweat and saliva brown or reddish-black. This is harmless, although it can stain clothes. It is not blood and does not need treatment.

What is "wearing off" with levodopa?

After some years, each dose may stop working before the next is due, so symptoms return between doses. Tell your neurologist – options include smaller doses more often, a CR tablet, or adding another medicine. Do not increase the dose yourself.

Can I stop Syndopa Plus if the shaking has stopped?

No. The shaking has stopped because the medicine is working. Stopping levodopa suddenly can cause a dangerous withdrawal reaction with high fever, stiff muscles and confusion. Any change must be made slowly under your doctor's guidance.

Does levodopa cause hallucinations?

It can, especially in older people and at higher doses – seeing people or animals that are not there, or becoming suspicious. Report this to the neurologist promptly; the dose or other Parkinson's medicines may be adjusted. Do not treat it with ordinary anti-psychotic tablets, which worsen Parkinson's.

Can I take iron tablets with Syndopa Plus?

Not at the same time. Iron binds levodopa in the gut and reduces its absorption. Take iron supplements at least 2 hours apart from the levodopa dose.

Is levodopa-carbidopa available at Jan Aushadhi stores?

Jan Aushadhi lists levodopa + carbidopa tablets in some strengths, but the exact 100 mg + 25 mg strength is not always stocked. It is a Schedule H medicine, so carry your prescription and check the strength on the strip matches what your neurologist wrote.
