Uses
- Chronic gout – lowering uric acid to prevent attacks, in people who cannot take allopurinol or in whom it did not work.
- High uric acid with tophi (uric acid lumps) or uric acid kidney stones.
- Preventing uric acid rise during chemotherapy for some blood cancers, under a specialist.
How to take it
Take one 40 mg tablet once a day, with or without food, at the same time each day. Your doctor may increase to 80 mg after 2 to 4 weeks if uric acid is still above target. Gout attacks can increase in the first few months as crystals dissolve; your doctor will usually give colchicine or a painkiller for the first 3 to 6 months – keep taking febuxostat through a flare. Do not start it during an acute attack. It is for long-term use; stopping lets uric acid rise again. Tell your doctor straight away about any chest pain, breathlessness or rash.
Side effects
Common
- Gout flares in the first months.
- Nausea, diarrhoea.
- Abnormal liver blood tests.
- Headache, mild rash.
- Joint pain, swelling of the feet.
Serious
Stop and get medical help urgently if you notice any of these.
- Chest pain, breathlessness, irregular heartbeat, one-sided weakness or slurred speech – febuxostat is linked with a higher risk of heart-related death in people with existing heart disease.
- Yellow skin or eyes, dark urine (liver injury).
- Severe skin reaction with blisters, peeling or fever.
- Serious allergic reaction.
Safety information
Alcohol
Keep alcohol low – it raises uric acid and triggers gout attacks, and adds to liver strain.
Pregnancy
Not recommended in pregnancy or breastfeeding; tell your doctor.
Kidney problems
Can be used in mild to moderate kidney disease without dose change; in severe kidney disease your doctor will decide.
Liver problems
Use with caution; liver tests are checked before starting and if symptoms appear.
Children
Not for use under 18.
Missed dose
Take it as soon as you remember unless the next dose is nearly due, then skip it. Do not double up.
Interactions
- Azathioprine and 6-mercaptopurine – never combine; febuxostat causes dangerous rises in their levels.
- Theophylline – levels may rise; tell your doctor.
- Thiazide diuretics (hydrochlorothiazide) and low-dose aspirin – raise uric acid and work against treatment.
- Other uric-acid lowering drugs (allopurinol) – not combined; one or the other.
Frequently asked questions
Febutaz 40 before or after food?
Either – febuxostat works the same with or without food. Take it at the same time every day and drink plenty of water.
Febutaz 40 vs Zyloric (allopurinol) – which is better for gout?
Allopurinol is the usual first choice. Febuxostat is used when allopurinol causes rash or does not lower uric acid enough, or in kidney disease. It is linked with more heart risk in people with existing heart disease, so the choice is your doctor's.
Why is my gout flaring after starting Zurig 40?
As uric acid drops, crystals in the joints dissolve and can trigger attacks for the first few months. Keep taking febuxostat and use the colchicine or painkiller your doctor prescribed.
Can I take Febutaz 40 with alcohol?
Keep alcohol minimal. Beer and spirits raise uric acid and bring on attacks, and alcohol adds to the strain on the liver.
Febutaz 40 vs Febutaz 80 – when is the higher dose needed?
Most people start on 40 mg. If the uric acid level is still above target after a few weeks, your doctor may increase to 80 mg. Do not change the strength yourself.
Is febuxostat safe for the heart?
Febuxostat carries a warning about a higher risk of heart-related death in people who already have heart disease. Tell your doctor about any heart attack, stroke or angina, and report chest pain or breathlessness immediately.
Can I stop Febutaz once my uric acid is normal?
No. Uric acid rises again within weeks of stopping and attacks return. Gout treatment is long-term; your doctor will review doses with regular blood tests.
Can Febutaz 40 be taken in kidney disease?
Yes, it is often preferred over allopurinol in mild to moderate kidney disease because no dose change is needed. In severe kidney disease your doctor will decide.
