Uses

  • Gout – lowering uric acid to prevent repeated attacks and joint damage.
  • Preventing uric acid kidney stones.
  • Preventing a rise in uric acid during chemotherapy for some cancers.
  • High uric acid that is causing symptoms, where diet alone has not helped.

How to take it

Take it once a day after food with a full glass of water, and drink plenty of fluids through the day. Treatment usually starts at a low dose (100 mg) and is increased every few weeks based on your uric acid blood test until the target is reached. Gout attacks can become more frequent in the first weeks – your doctor may give colchicine or a painkiller alongside – but keep taking allopurinol; stopping and restarting makes flares worse. Do not start it during an acute attack unless you are already on it. Stop and see a doctor at once if you develop any rash.

Side effects

Common

  • Skin rash (stop and contact your doctor – it can be the start of a serious reaction).
  • Nausea, diarrhoea.
  • More gout attacks in the first months.
  • Drowsiness or dizziness.

Serious

Stop and get medical help urgently if you notice any of these.

  • Severe skin reactions with blisters, peeling, mouth ulcers or fever (Stevens-Johnson syndrome / DRESS) – emergency.
  • Yellow eyes, dark urine (liver injury).
  • Unusual bleeding, bruising or frequent infections (blood cell problems).
  • Severe allergic reaction with swelling of the face or throat.

Safety information

Alcohol

You can drink in moderation, but alcohol – especially beer – raises uric acid and triggers gout attacks, so keep it low.

Pregnancy

Not usually used in pregnancy unless clearly needed. It passes into breast milk; discuss with your doctor.

Kidney problems

The dose is reduced in kidney disease and increased very slowly. Your doctor will check creatinine before and during treatment.

Liver problems

Use with care in liver disease; liver tests may be monitored.

Children

Only used in children for uric acid problems during cancer treatment, under a specialist.

Missed dose

Take it as soon as you remember unless the next dose is nearly due, then skip it. Do not double the dose.

Interactions

  • Azathioprine and 6-mercaptopurine – allopurinol greatly increases their levels; the dose must be cut sharply or the combination avoided.
  • Warfarin – bleeding risk may rise; INR needs checking.
  • Amoxicillin or ampicillin – higher chance of skin rash.
  • Diuretics like hydrochlorothiazide and ACE inhibitors (enalapril, ramipril) – higher risk of allergic reactions and kidney effects.
  • Theophylline – levels may rise.
  • Aspirin in low dose – can raise uric acid; tell your doctor.

Frequently asked questions

Zyloric 100 before or after food?

After food, with a full glass of water. Taking it with a meal reduces nausea, and drinking plenty of water through the day helps flush uric acid.

Why did my gout get worse after starting Zyloric?

As uric acid levels fall, crystals already in the joints can shift and trigger attacks in the first few months. This is expected; keep taking allopurinol and use the colchicine or painkiller your doctor prescribed for flares.

Can I take Zyloric 100 with alcohol?

A small amount is allowed, but alcohol – beer in particular – raises uric acid and brings on attacks, so it works against the treatment.

Zyloric 100 vs Zyloric 300 – which should I take?

Most people start at 100 mg and the dose is increased in steps based on blood tests until uric acid reaches the target. Do not change the strength yourself.

I got a rash on Zyloric – what should I do?

Stop the tablet and see a doctor the same day. A rash with allopurinol can be the start of a rare but dangerous skin reaction, which is more common in people of Indian and East Asian origin.

Do I need to take allopurinol for life?

Usually yes. Gout is a long-term condition and uric acid rises again when you stop. Your doctor will check levels once or twice a year.

Can I take Zyloric during a gout attack?

If you are already on it, continue. If you have not started yet, doctors usually wait until the attack settles, because starting during a flare can prolong it.

Is Zyloric safe if I have kidney disease?

It can be used, but at a lower starting dose with slow increases and regular blood tests. Tell your doctor your latest creatinine.
